Release of Information

If you wish to obtain information regarding a patient, the patient must provide written consent in accordance with privacy legislation.
Please see below a printable release of information form for the patient to complete and sign.Once completed, fax the document (Fax: 9372 7044) and we will be able to provide the corresponding records upon confirming this with the patient.

If you’re booking to see us as a New Patient, please see below a printable Patient Registration form.You may wish to print it out at home, fill it in and bring it with you. If not, you will be provided a copy to complete on arrival.
